For Transcripts:

Please mail a written request. Your letter must include your full name (and maiden name, if applicable, year of graduation,signature, address, phone number and the last four digits of your social security number. There is a $1.00 fee, cash, check or money order payable to Wheeling High School for each transcript. You must also provide the full name and address of the scholarship committee/college/university where the transcript is to be sent. Unofficial tgranscripts require the same information and processing fee and can be sent directly to your home.

Please note: ACT test scores are not included on the transcript. Go to act.org for test scores.

For Immunization Records:

Immunization requests require the same information above. There is a $3.00 processing fee for each copy and are only mailed to your home address.
